Jon Hammer

JON BRYANT HAMMER, 60, born in Rochester, NH 0n April 29, 1954 to the late Donald I and Margaret L Hammer, passed away on February 7, 2015, in Newport, RI. Jon was a client of Community Partners and participated in many of its programs in Rochester, Dover and Somersworth. Jon is survived by his brother and sister-in-law Donald and Linda Hammer of Newport, RI, his sister and brother-in law Patricia and Joseph Main of Slatington, PA, six nieces, and one nephew. Jon is also survived by his home care giver and her husband, Diane and George Chalifour of Rochester, who cared for Jon for nearly 20 years. A Memorial Service will be held for Jon at the Edgerly Funeral Home in Rochester on April 25 at 10:00 am, followed by interment at Rochester Cemetery. Contributions in lieu of flowers contributions may be made to Community Partners of Rochester.
